The transfer of care from the pediatric team to the team of adult doctors needs to be prepared.

During this so-called "transitional" period, repeated explanations and consultations made only with the adolescent must allow an empowerment of the teenager versus his/her family, to establish a life project (professional integration, pregnancy, sexuality) and address specific points such as knowledge of the disease, treatments and their complications, prevention of smoking, contraception and therapeutic adherence. When changing teams, all medical information is transmitted by the pediatrician to the new team. This change occurs when the teenager and his parents are ready, usually around 16-18 years old.
